Job Title: Home Care Coordinator

**Job Summary:**

We are seeking a skilled Home Care Coordinator to join our team at King Premium Care. As a Home Care Coordinator, you will play a vital role in ensuring our clients receive the highest standard of care in the comfort of their own homes.

**Key Responsibilities:**

**Care Plan Development:** Collaborate with clients, families, and healthcare providers to develop and implement individualized care plans that meet their unique needs and goals.

**Team Coordination:** Schedule and supervise care staff, ensuring proper training and adherence to care plans.

**Client and Family Support:** Provide ongoing support to clients and their families, addressing any concerns or needs and ensuring satisfaction with services.

**Quality Assurance:** Monitor the quality of care provided, conducting regular assessments and audits to ensure compliance with company standards and regulatory requirements.

**Documentation and Reporting:** Maintain accurate and up-to-date records of client care, staff schedules, and other relevant documentation.

**Problem Resolution:** Address and resolve any issues or conflicts that arise, ensuring timely and effective solutions.

**Communication:** Serve as a primary point of contact for clients, families, and care staff, fostering open and effective communication.

**Qualifications:**

**Education:** CNA, Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field preferred.

**Experience:** Minimum of 2-3 years of experience in home care, healthcare coordination, or a related field.

**Skills:** Strong organizational, interpersonal, and problem-solving skills. Proficient in using electronic health record (EHR) systems and other relevant software.

**Certifications:** Relevant certifications (e.g., CPR, First Aid) are a plus.
