Wound Care Clinical Coordinator

As a Wound Care Clinical Coordinator, you will be responsible for providing case management to patients, organizing and prioritizing clinical responsibilities, and collaborating with healthcare providers to ensure high-quality patient care.
Key Responsibilities- Provides case management to patients, including assessments, treatments, and patient education related to wound care management.
- Organizes and prioritizes clinical responsibilities of clinical staff, including medical assistants and wound care technicians.
- Collaborates with healthcare providers, key hospital personnel, and Wound Care Center Providers to ensure effective patient care.
- Identifies areas for improvement and implements changes to ensure compliance with Health Insurance Portability and Accountability Act (HIPAA) and Department of Health (DOH) regulations.
- Communicates lab and test results to patients and provides education on wound care management.
- Develops and maintains a system for tracking patient care and ensuring that patients receive appropriate tests and therapies.
- Provides staff development and education, including in-services and individual instruction and orientation.
- Ensures that the clinic adheres to all relevant healthcare regulations, standards, and guidelines.
- Implements quality assurance measures to monitor and improve the quality of patient care.
- Conducts regular audits and assessments to identify areas for improvement and address any compliance issues.
- Current active Medical Assistant certification in the state of practice through an accredited certified medical assistant program.
- Two (2) years of direct patient care experience required.
- Current Basic Life Support (BLS) certification is required.
- Clinical experience with a focus on wound care preferred.
- Experience working in an acute care or rehabilitation setting preferred.
- Proficient in all responsibilities related to Case Management.
- Strong clinical assessment and critical thinking skills.
- Effective communication and interpersonal skills.
- Ability to work collaboratively with the team.
