Key Responsibilities
  • Provide case management to patients, conducting assessments and implementing wound care management protocols.
  • Organize and prioritize clinical responsibilities, ensuring seamless coordination with medical assistants and wound care technicians.
  • Collaborate with healthcare providers, hospital personnel, and medical directors to address clinical and patient needs.
  • Identify areas for improvement and implement process enhancements, adhering to HIPAA and DOH regulations.
  • Communicate lab and test results to patients, ensuring timely and effective care.
  • Develop and maintain a system for tracking patient care considerations, ensuring timely scheduling of tests and therapies.
  • Engage in staff development, providing in-services and individual instruction to enhance clinical skills.
  • Recognize and reward staff for outstanding performance, promoting a culture of excellence.
  • Ensure compliance with healthcare regulations, standards, and guidelines, implementing quality assurance measures to monitor and improve patient care.
  • Conduct regular audits and assessments to identify areas for improvement and address compliance issues.
  • Collaborate with the Medical Director to achieve Wound Care Center quality indicators.
  • Perform patient care duties as needed, ensuring seamless operations and steady progress towards established goals.
  • Establish and maintain open communication with staff, identifying problems and communicating solutions to appropriate personnel and departments.
  • Identify opportunities for process improvement and efficiency gains, partnering with the Clinic Manager and Medical Director to implement changes.
  • Implement new technologies and workflows to enhance patient care delivery and operational effectiveness.
  • Ensure a safe patient and work environment by maintaining incident reports, room hygiene logs, and inventory/medication expiration date logs.
  • Monitor Wound Care Tech/Medical Assistant tasks related to patient inquiries, complaints, and feedback.
  • Respond to patient complaints and questions, maintaining employee counseling records for clinical staff.
  • Monitor clinical skills of clinical staff, delivering clinical education training programs as needed.
  • Cover the front office as needed, responding to and/or replacing back office call outs.
  • Monitor patient flow and assist clinically as needed, based on wait times and needs of the clinic.
  • Assist the management team with maintaining equipment and calibration records.
  • Monitor inventory levels and order supplies as needed to ensure adequate stock levels.
  • Participate in Outpatient Medicine and Vituity projects to accomplish the strategic plan of Vituity.
  • Orient and educate staff as needed on expectations, clinic policies, and procedures.
Requirements
  • Current active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice is required.
  • Two (2) years of direct patient care experience required.
  • Current Basic Life Support (BLS) certification is required.
  • Clinical nursing experience with a focus on wound care preferred.
  • Experience working in an acute care or rehabilitation setting preferred.
  • Proficient in all responsibilities related to Case Management.
  • Strong clinical assessment and critical thinking skills.
  • Effective communication and interpersonal skills, with the ability to develop and maintain effective relationships with patients, families, staff, providers, and all other members of the health team.
  • Ability to work collaboratively with our team.
  • Proficient with advanced wound care technologies and techniques.
  • Solid understanding of medical insurance plans and office procedures.
  • In-depth knowledge of medical documentation.
  • Strong clinical assessment and critical thinking skills.
  • Ability to demonstrate leadership skills to delegate and provide direction/guidance to others on the team.
  • Able to stay organized and flexible in a work environment with multiple assignments and changing priorities.
  • Ability to uphold Health Insurance Portability and Accountability Act (HIPAA) requirements.
  • Highly organized and able to manage multiple priorities.
  • Ability to identify strategies to improve patient satisfaction and outcomes.
