Care Management Supervisor

At Universal Healthcare Resources, we are seeking a highly skilled Care Management Supervisor to join our team. As a key member of our healthcare team, you will be responsible for providing direct care management services to a defined patient population, while also supervising a team of case management and social work staff.
The ideal candidate will have a Master's degree in Social Work or a related field, and a minimum of 5-7 years of experience in a healthcare environment, with at least 2 years in a leadership role. A licensed social work credential is preferred, and certification in CCM/ACM is a plus.
Responsibilities will include:
- Maintaining a caseload and supporting departmental activities for members of the Care Management team
- Working with patients, families, and healthcare teams to develop comprehensive care plans
- Assessing patient needs and developing care management plans to address those needs
- Providing supervision and guidance to department staff, including orientation, mentoring, coaching, and evaluation
- Coordinating training and education programs for staff, and maintaining employee files
- Communicating and interacting with internal and external customers, including healthcare facilities, community social service agencies, and third-party payers
