We are seeking a highly skilled and experienced Clinical Coordination Manager to join our team at Heartland Community Health Center.

Key Responsibilities:
  • Develop and implement effective referral coordination processes, ensuring seamless patient care
  • Lead and support the clinical team in maintaining accurate records, tracking referrals, and communicating with healthcare providers
  • Collaborate with patients, families, and healthcare providers to ensure informed decision-making and timely follow-up
Requirements:
  • Bachelor's degree in a related field or equivalent experience; medical office experience preferred
  • Strong leadership skills, with a focus on teamwork, communication, and problem-solving
  • Excellent communication and interpersonal skills, with a focus on empathy and patient-centered care

We offer a competitive salary range of $60,000 - $80,000 per year, based on experience, and a comprehensive benefits package.
