About the Role

This dynamic position focuses on transition planning and Utilization Review, ensuring seamless care for patients. As a Unit-based Case Manager, you will be responsible for initial assessments, developing transition plans, participating in multidisciplinary rounds, and conducting utilization reviews.

Responsibilities
  • Assess patient needs and develop effective transition plans
  • Collaborate with interdisciplinary teams to optimize care coordination
  • Conduct thorough utilization reviews to ensure appropriate resource allocation
  • Advocate for patients' needs and preferences

This role requires strong critical thinking skills, excellent communication abilities, and a willingness to learn in a fast-paced environment.

Requirements
  • Current Nursing License in the State of MA required
  • Three or more years of clinical experience in healthcare delivery systems
  • Completion of an accredited Nursing program
  • Culturally competent and able to work effectively with diverse populations
Preferred Qualifications
  • Case Management or Home Care Discharge Planning experience

A competitive salary of $85,000 - $110,000 per year, depending on experience, is offered for this position. Mass General Brigham provides a comprehensive benefits package, including medical, dental, vision, life insurance, and employee assistance programs.
