Job Overview

We are seeking an experienced Registered Nurse to join our Case Management team as a Nurse Case Manager. This role will focus on transition planning and utilization review, with a strong emphasis on assessment and development of individualized care plans.

Requirements

To be successful in this role, you will need:

  • A current Nursing License in the State of MA
  • Three or more years of clinical experience in a healthcare delivery system
  • Completion of an accredited Nursing program
  • Culturally competent communication skills
Preferred Qualifications

While not required, we prefer candidates with experience in case management, home care discharge planning, and InterQual software training.
