Mass General Brigham seeks a dedicated Healthcare Transition Coordinator to facilitate seamless patient care in Lynn, Massachusetts.

About the Role

This is an ongoing opportunity for 32 hours per week with an 8-hour day shift. The ideal candidate will possess exceptional communication and interpersonal skills, with experience working in a fast-paced healthcare environment.

We take a comprehensive approach to determining base pay, considering your skills, relevant experience, education, certifications, and other critical factors. Based on this analysis, we estimate the salary range to be between $65,000 - $90,000 annually, depending on qualifications.

The successful candidate will work closely with our experienced Case Management team, focusing on transition planning and Utilization Review. As a unit-based case manager, you will assess patients' needs and develop individualized transition plans, ensuring a smooth handover of care.

You will participate in multidisciplinary rounds, conduct utilization reviews, and advocate for patients' needs. Your ability to delegate tasks to Case Management assistants will be essential in maintaining efficient workflows.
