Integrated Mental Health Therapist II

Sea Mar Community Health Centers is seeking a highly skilled Integrated Mental Health Therapist II to join our team. As a core member of the Clinical Care Team, you will work within a medical setting to provide diagnostic assessments, referrals for psychiatric services, and case management in coordination with other teams or parties involved in treatment.
The ideal candidate will have a strong understanding of chronic conditions, including substance use disorders and behavioral health concerns that contribute to a patient's inability to adequately address their medical needs. You will also have an understanding of chronic medical conditions that can lead to depression and other mental health concerns.
In this role, you will educate staff on the process for mental health service referrals and on psycho-educational topics, such as suicide risk assessment and behavior modification. You will maintain documentation of mental health services and follow through in patient charts with strong emphasis on communication with the care team located in the medical clinic.
Responsibilities- Provide brief, strength-based, and solution-focused services to adults, children, and families
- Assist with emotional and behavioral stabilization, including suicide prevention and de-escalation of patients
- Refer patients for crisis services when needed
- Provide motivational-based wellness coaching and help patients create self-management goals
- Work with patients to increase their level of motivation to meet goals
- Educate staff on the process for mental health service referrals and on psycho-educational topics
- Maintain documentation of mental health services and follow through in patient charts
- Master's degree from an accredited college or university in psychology, counseling, or social work, which includes coursework in psychological diagnostics
- One to three years of experience in the field, which can be obtained before or after completion of a Master's degree
