Position Type: Part-Time (20 hours/week)

We are looking for a Service Coordinator to manage service coordination and case management for our resident members within CSI Support & Development co-ops. The primary goal of this role is to empower residents to maintain their independence and enhance their ability to live comfortably within the co-op environment.

This position necessitates proven experience in organizing community services tailored for older adults. The ideal candidate will possess strong interpersonal skills, along with proficiency in program planning, general office tasks, and computer applications. Candidates who are organized, adaptable, knowledgeable, and fluent in Spanish will excel in supporting our resident members.

Key Responsibilities:
  • Guide and inform members and their families about essential services for independent living.
  • Evaluate the needs of resident members regarding health, psychological, and social services.
  • Assist members in creating informal support networks within their community.
  • Educate residents, their families, and staff about available community resources.
  • Act as a liaison between community agencies, service providers, and resident members.
  • Facilitate the fulfillment of needs while promoting healthy independence.
  • Collaborate with members, council, and co-op committees.
  • Build relationships with local service providers.
  • Monitor the quality and effectiveness of services delivered.
  • Plan and execute engaging educational and group programs.
  • Assist in mediating conflicts among resident members when necessary.
  • Maintain a comprehensive directory of community services for residents and staff.
  • Keep individual member files updated with relevant contact information.
  • Pursue ongoing education related to services for the elderly.
  • Ensure accurate reporting and distribution of information to meet deadlines.
Qualifications:
  • Bachelor's degree or higher in social work, psychology, gerontology, counseling, or a related field, with at least 1 year of experience working with older adults in a community setting; or an associate's degree with 3+ years of experience in case management or service coordination.
  • Fluency in Spanish is a plus.
  • Strong oral and written communication skills.
  • Familiarity with social work terminology and reporting processes.
  • Knowledge of senior community resources.
  • Ability to work effectively in a multi-cultural environment.
  • Experience in health & wellness and grief counseling is desirable.
  • Proficiency in Microsoft Office Suite.
  • Ability to work independently.
Working Conditions:
  • Ability to engage with challenging clients.
  • May require entering apartments in various conditions.
Physical Requirements:
  • Must be able to lift or push up to 20 pounds.
  • Ability to sit for extended periods.
  • Frequent use of keyboard and phone.
