Job OverviewEmployment Type: Full time Shift: Day Shift Role Summary:

This position is integral to the healthcare team, focusing on delivering comprehensive care management and coordination within outpatient settings. The selected candidate will engage with patients across various insurance plans, including Medicaid, Medicare, and commercial insurance, guiding them through their healthcare journey.

The role emphasizes the provision of tailored access to both physical and behavioral health services, as well as community support resources. The objective is to ensure that patients receive appropriate care in a timely manner, while also empowering them and their families to take an active role in their health management through effective education and shared decision-making.

Key Responsibilities:

  1. Develop and coordinate individualized care plans that address the unique needs of patients and their families.
  2. Monitor and manage patient progress, ensuring that all aspects of care, including wellness, medical treatment, and social services, are effectively integrated.
  3. Utilize population health tools to track wellness goals and enhance patient outcomes, focusing on chronic illness management.
  4. Collaborate with a multidisciplinary team, including social workers and pharmacists, to create patient-centered care strategies.
  5. Implement evidence-based practices to optimize care plans and achieve desired health outcomes.
  6. Engage in professional development activities to enhance personal skills and contribute to the growth of colleagues and students.

Qualifications:


* Minimum Education:

Required: Bachelor's Degree in Nursing
Preferred: Master's Degree in Nursing


Specify degrees: BSN

Minimum Experience:


Required: 3-5 years of nursing experience
Preferred: 6-10 years of nursing experience
