Pathways Vermont is seeking a highly skilled and compassionate Assertive Community Treatment (ACT) Team Coordinator to join our dynamic team. As a key member of our interdisciplinary team, you will provide flexible, community-based services to promote the stability, recovery, and community reintegration of individuals experiencing mental health challenges, histories of homelessness, and substance use issues.
Key Responsibilities- Relationship Building and Service Delivery: Engage in relationship building and rapport building with clients, providing services from a client-centered, harm reduction, and strengths-based approach.
- One-to-One Support and Counseling: Provide one-to-one support and counseling on life skills, housing concerns, and health maintenance, including medication support, housekeeping, and assisting clients to manage their finances.
- Housing Navigation and Support: Assist clients in navigating the housing path, including viewing units, assistance with moving in, life skill development, and landlord/client disputes.
- Community Resource Coordination: Coordinate and monitor referrals to community services, advocate for client participation, and manage contacts with outside agencies.
- Knowledge of Community Resources: Develop a working knowledge of relevant community resources and demonstrate a willingness to explore new or unfamiliar resources as necessary.
- Housing First Framework: Ability to work within a Housing First, client-centered, harm reduction framework.
- Patience and Compassion: Patience, creativity, flexibility, compassion, and sensitivity to persons with disabilities and other marginalized populations.
- Understanding of Mental Health Challenges: Good understanding of the issues facing persons with mental health challenges or substance use issues, particularly as related to meeting basic needs of food, clothing, shelter, and advocacy.
- Problem-Solving Skills: Excellent independent problem-solving skills and demonstrated ability to intuitively approach challenging situations.
- Valid Driver's License: Valid driver's license, a reliable personal vehicle available for work purposes, and current automobile insurance required.
- Competitive Compensation: $20-$21 starting hourly wage, with up to 8.5% increase available after 1 year employment.
- Hiring Bonus: $3000 hiring bonus available.
