Job Responsibilities:
  • Deliver home-based medical care to patients in an assigned geography
  • Collaborate with interdisciplinary teams to improve patient outcomes
  • Develop and implement patient care plans
  • Communicate effectively with patients, caregivers, and healthcare professionals
Requirements:
  • Completion of a Nurse Practitioner accredited program
  • Current NP license
  • Current state RN license
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
Compensation:

The estimated salary for this position is $130,000 - $160,000 per year, depending on experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, including student loan repayment and career advancement opportunities.
