LPN-Palliative Care Specialist

As a skilled LPN, you will provide high-quality care to patients in their homes, focusing on palliative care. This role requires a compassionate and detail-oriented individual who can work independently and as part of a team.
Responsibilities:
Perform visits for assigned patients and families in their homes, telephonically, or via video.
Report changes in patient condition/symptoms and coordinate care with oversight from the RN/advanced practice RN.
Engage patients and caregivers in discussions about their goals of care, advance directives, and participate in the development of treatment plans based on the patient's goals.
Participate in assisting with and giving direct patient care in conjunction with the rest of the clinical care team to ensure interventions are aligned with patients' goals.
Advocate for the patient and caregivers in liaison with other care providers to ensure care is both coordinated and consistent with the patient's goals.
Educate patient/family on the trajectory of illness, and use of medications.
Consult with RN/advance practice RN and seek guidance as necessary.
Comply with policies, procedures, and regulatory mandates.
Qualifications:
Graduate of an accredited licensed practical nursing program or accredited school of vocational nursing.
Current, unencumbered license to practice as a licensed practical nurse specific to the state you are assigned to work.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health, palliative, or hospice setting.
