Palliative Care Specialist

VIA Health Partners is seeking a compassionate and skilled Palliative Care Specialist to join our team. As a Palliative Care Specialist, you will provide high-quality, patient-centered care to individuals with advanced life-threatening illnesses.
Key Responsibilities:
- Provide comprehensive medical care to patients with advanced life-threatening illnesses
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Provide education and support to patients and their families
- Stay up-to-date on current trends and best practices in palliative care
Requirements:
- Master's degree in nursing
- Current, active, and unrestricted license as a Registered Nurse (RN) in North Carolina
- Official recognition as a Nurse Practitioner from the North Carolina Board of Nursing
- Successful board certification in a specialty within 2 years of hire
- Current and unrestricted DEA and Basic Life Support required
