Key Responsibilities

  • Conduct regular face-to-face case management sessions and quarterly home visits.
  • Provide education on topics related to successful housing retention and relapse prevention.
  • Support clients in developing independent living skills and creating individualized service plans.
Requirements
  • Bachelor's degree in social work, human services, or a related field.

  • Strong knowledge of housing market, housing programs, and resources.
  • Ability to communicate with and relate to a diverse group of people.
