Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Canton, Ohio.

About the Job

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in their homes. As an Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patient's level of functioning and develop individualized plans of care to improve range of motion, muscle strength, and coordination.
  • Teach patients and their families on proper use of equipment and provide education on disease management and prevention.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordinate services.
  • Document patient progress and maintain accurate records.
Requirements
  • Current license as an Occupational Therapist in the state of Ohio.
  • Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance.
  • Ability to lift and move patients as needed.
