Home Health Occupational Therapist

We are seeking a skilled Occupational Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Occupational Therapist, you will provide high-quality care to patients in their homes, helping them to regain independence and improve their quality of life.
Key Responsibilities- Develop and implement individualized plans of care for patients, focusing on occupational therapy goals and objectives.
- Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas for improvement and developing strategies to address them.
- Provide hands-on therapy sessions, using a variety of techniques and modalities to promote patient progress and independence.
- Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and coordination.
- Document patient progress and outcomes, maintaining accurate and detailed records of treatment plans and results.
- Current licensure as an Occupational Therapist in the state of employment.
- Minimum one year of experience as an Occupational Therapist in a home health or acute care setting.
- Valid driver's license and reliable transportation.
- Ability to lift, bend, and move patients as needed, with or without reasonable accommodation.
