We are seeking a qualified Care Management professional to join our team at Coastal Horizons Center, Inc.
Job Title: TASC Care Manager
Location: Orange County, NC
Hours: 40 hours/week, Full Time, M-F, Non-Exempt
The successful candidate will administer psycho-social screenings/assessments, coordinate linkage to behavioral health treatment, medical, educational, and vocational services as needed, and provide ongoing professional care management services for the justice-involved population.
Qualifications:
- Education: High school diploma or higher
- Certifications: Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB)
- Experience: Knowledge of recovery, harm reduction, and wellness-based concepts. May have lived experience with recovery and/or the justice system
- Additional Skills: Proficient computer skills including the ability to navigate the internet, word processing and spreadsheet applications, and basic email platforms. Bilingual Spanish/English speaking skills are a plus
Benefits: Salary Range for this position is $37,000-$42,000 commensurate with experience, medical/dental insurance, life insurance, disability insurance, retirement savings plan/401K, paid time off programs.
