Care Manager Job Summary
UNC Medical Center is seeking a skilled Care Manager to join our team. As a Care Manager, you will be responsible for providing ongoing support and expertise to patients through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
Key Responsibilities:
- Provide ongoing support and expertise to patients through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
- Enhance the quality of patient management and satisfaction by integrating case management, utilization review, and discharge planning.
- Review work list daily to prioritize patients and identify new admissions.
- Conduct and document assessments and care plans in Epic per departmental guidelines.
- Participate in daily care management touchpoints per established protocols.
- Communicate with Care Management Assistant to share priorities, if indicated.
- Attend and actively participate in meetings to provide and receive information on patient progression.
- Alert the care team to concerns that may impact anticipated discharge and assist with discharge readiness.
- Modify discharge plans based on shared information and assist with identifying the expected discharge date.
- Discuss updates and action items with the Utilization Manager and Social Worker after meetings, as necessary.
- Attend weekly meetings to problem-solve issues with complex patients and monitor cases.
- Identify high-risk cases that require escalation.
- Collaborate with the multidisciplinary team concerning barriers to discharge and psychosocial concerns.
- Coordinate family meetings and provide education on community resources.
- Communicate medical milestones for transition to patients and families.
- Monitor all observation patients to ensure appropriate care progression.
- Ensure patient's understanding and agreement with the discharge plan.
- Refer administrative tasks to the Care Management Assistant and consult with the Social Worker and Utilization Manager as necessary.
Requirements:
- Nursing Diploma or ADN/ASN from an accredited school of Nursing.
- Two years of healthcare experience as a Registered Nurse.
