Care Manager RN Job Description

UNC Health Care is seeking a skilled RN Care Manager to join our team. As a Care Manager RN, you will play a vital role in providing comprehensive and coordinated care to patients within our healthcare system.

Key Responsibilities:

  • Conduct comprehensive care manager assessments to evaluate patients' functional abilities, cognitive status, and psychosocial support systems.
  • Develop and implement individualized care plans that align with patients' goals, preferences, and values.
  • Collaborate with healthcare providers, patients, and their families to ensure seamless care transitions and continuity of care.
  • Communicate and coordinate with payers, insurance companies, and utilization management teams to optimize reimbursement and facilitate timely approvals for necessary care and services.

Requirements:

  • Graduation from a state accredited school of professional nursing.
  • Registered Nurse with a valid license to practice in North Carolina.
  • Minimum of 2 years of experience as a registered nurse.
