Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at V R Della Infotech Inc. in a Long Term Care (LTC) setting. As an LPN, you will provide direct nursing care to residents under the supervision of an RN Clinical Manager. Your primary responsibilities will include making frequent rounds to evaluate resident status, passing medication as required, providing wound care, and documenting care provided.

Key Responsibilities
  • Provide direct nursing care to residents, including making frequent rounds to evaluate resident status and passing medication as required.
  • Provide wound care and document care provided.
  • Supervise nursing assistants and ensure assignments are completed.
  • Assume the position of team leader of unit and act as liaison between nurses, nursing assistants, and clinical manager.
  • Plan nursing care on unit assigned for 8 hours, up to 12 hours, and maintain high nursing standards.
  • Assist in performing nursing assessments, including focused screening, assessments, and observational assessments.
  • Collect data and information and communicate outcomes of data collection process in a timely manner.
  • Contribute to the development of the nursing care plan and attend care plan meetings.
  • Communicate with medical providers and family members regarding condition changes, medication changes, and behavioral concerns.
  • Notify MD of change in residents' condition and notify funeral director in case of resident death.
  • Make out transfer forms and complete all arrangements for transfer.
  • Make up work schedule for nurse assistants on unit and explain special needs of residents.
  • Assist in giving ADL nursing care when needed.
  • Set up, administer, and chart medications in ECS program.
  • Do treatments and chart according to electronic TAR.
  • Initiate starting up of machinery such as nebulizers, oxygen, suction, etc.
  • Do daily charting as assigned in ECS and as needed.
  • Supervise care of critical residents and help with care as time permits.
  • Assist clinical manager with evaluation of nursing assistants and new LPNs.
  • Take part in nursing conferences, neighborhood teaching, and team meetings/in-service programs.
  • Shall accept responsibility of shift supervisor, when assigned, to care, keys, change schedules, as needed, and call clinical manager on 2nd or 3rd shift.
  • Nurses are to take breaks and lunches when they can but must cover for each other on the floor.
  • Shall accept all related duties which may develop concerning the above functions.
  • Refer to functions and suggested procedure routines attached for complete description.
  • Participate/support Eden process.
  • Responsible and accountable for the quality of nursing care given.
  • Responsible and accountable for delegated activities.
  • Responsible and accountable for following the standards of nursing conduct or practice.

Minimum Qualifications
  • Graduate Licensed Practical Nurse licensed in Michigan Required.
  • Bachelor's Preferred.
  • BLS Certification - Required.
  • Caregiving: 1 LTC year Preferred.
  • New Grad welcome.
  • Can provide Covid Vaccination Card but not needed - If exempt please provide in additional note section and the client will provide form to be filled out where you will include a statement from the candidate.
  • Must be able to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly and/or emotionally upset.
  • Must be able to communicate in English.
  • Must be detail oriented and have excellent literacy and writing, documentation, communications and interpersonal abilities.
  • Must be able to use tools and equipment required.
  • Other job duties as assigned.
