Position Highlights:
- Support a designated provider team to enhance patient access and quality of care.
- Work schedule: Monday to Friday.
Role Responsibilities:
The role involves delivering patient care in alignment with established practice standards. This includes expanding patient access through both in-person and virtual consultations, as well as collaborating with team members to address patient needs effectively.
- Collaboration: Work closely with assigned providers to ensure the highest level of patient care.
- Clinical Duties: Conduct thorough patient assessments, determine diagnoses, and formulate treatment plans.
- Patient Education: Provide education to patients managing chronic conditions or seeking wellness care.
- Referral Management: Facilitate referrals to specialty care when necessary.
- Consultation: Engage with physicians or experienced APCs for complex cases outside of the APP's scope.
Qualifications:
- Completion of a Master's or PhD program in Nurse Practitioner or Physician Assistant.
- New graduates are welcome to apply.
Preferred Experience:
- At least one year of experience in Primary Care as a Nurse Practitioner or Physician Assistant.
Licensure Requirements:
- Current RN license (for NP) in the states of MN and/or WI.
- NP or PA license in MN and/or WI.
- DEA certification is mandatory.
- BLS certification required within 180 days of employment.
Physical Requirements:
Medium Work: Ability to lift up to 32 lbs occasionally and up to 25 lbs frequently, adhering to safe patient handling policies.
