Care Services Liaison

JOB SUMMARY
The Client Services Coordinator plays a vital role in facilitating the daily intake and referral processes to ensure clients receive the necessary services. This position involves coordinating in-home care requirements while adhering to Federal and State regulations, as well as Managed Care Organization (MCO) program guidelines. The coordinator is responsible for managing a caseload of clients enrolled in Medicaid programs and conducting regular assessments to adapt care plans as client needs evolve.
JOB DUTIES
- Oversee and manage a caseload of clients participating in Medicaid programs.
- Ensure clients' in-home care needs are met in accordance with Federal and State regulations, MCO program standards, and contractual obligations.
- Facilitate daily intake and referral processes to promote positive organizational growth.
- Encourage a collaborative and supportive office atmosphere.
- Assist in the daily operations and advocate for client needs.
- Demonstrate the ability to fulfill job responsibilities with minimal supervision.
- Engage with stakeholders regarding service delivery.
- Exhibit strong written and verbal communication skills.
- Adhere to all relevant legal requirements, standards, policies, and procedures.
- Show reliability and consistency in job performance.
- Utilize effective problem-solving and decision-making abilities.
- Display proficiency in computer applications.
- Manage complex service programs and special initiatives.
- Promote services to referral sources as necessary.
- Maintain the skills and knowledge required to coordinate workflow efficiently.
- Participate in ongoing professional development and training opportunities.
- Effectively prioritize tasks and manage multiple responsibilities.
- Deliver exceptional customer service to both internal and external clients.
- Represent the organization at various meetings and events.
- Assist clients and employees with enrollment in services and systems as required.
- Conduct necessary assessments and home visits as needed.
- Bilingual capabilities are preferred in various locations.
- Handle phone and video communications as required.
- Perform additional duties as assigned.
QUALIFICATIONS
- A Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or a related field is preferred.
- At least one year of experience working with individuals with disabilities or the elderly is preferred.
- A combination of education and relevant experience is acceptable.
- Bilingual proficiency in Spanish or Arabic is preferred.
The role typically takes place in an office setting, utilizing a computer, telephone, and other office equipment to fulfill responsibilities. The work environment noise level is consistent with that of a standard office. The incumbent may experience frequent interruptions throughout the workday. Regular tasks include sitting, talking, and listening; frequent use of repetitive hand motions; and the ability to stand, walk, reach, bend, or lift up to 20 pounds.
