Job Summary

US Renal Care is seeking a skilled Clinical Operations Manager - Value-Based Care to join our team. As a key member of our Value-Based Care team, you will play a critical role in ensuring the delivery of high-quality patient care and services.

Key Responsibilities
  • Value-Based Care Clinical Operations: Responsible for the clinical operations activities across the continuum of care, including assessing, implementing, coordinating, monitoring, and evaluating patient care.
  • Health Education and Treatment Decision Support: Provides health education and treatment decision support to patients, ensuring they have the knowledge and skills necessary to manage their chronic kidney disease.
  • Interdisciplinary Collaboration: Collaborates with patients, patient support networks, and interdisciplinary teams, including dialysis clinics, hospitals, and physician practices, to assess patient needs and develop comprehensive care plans.
  • Patient Visits and Outreach: Conducts onsite patient visits and outbound calls to assess patient health status, identify gaps or barriers in treatment plans, and provide education and support to patients and their families.
  • Consultation and Coordination: Coordinates with physicians and other healthcare professionals to support the quality and level of care provided for patients affected by chronic kidney disease/end-stage kidney disease.
  • Early Identification and Support: Assists with early identification and support for dialysis and transplant needs by working closely with nephrologists and the Value-Based Care team.
  • Patient Education and Self-Management: Provides patient education to assist with self-management, empowering patients to take an active role in their care.
  • Compliance and Regulatory Requirements: Knowledge of and compliance with federal, state, local laws, and regulations, including healthcare professional practice act requirements.
Requirements
  • Experience in value-based care, clinical operations, or a related field.
  • Strong knowledge of chronic kidney disease and end-stage kidney disease management.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Knowledge of healthcare regulations and compliance requirements.
