Job Summary

We are seeking an experienced Senior Hospice Nurse to join our team at Liberty Health in Durham/Burlington. This is a full-time position with a $10,000 sign-on bonus.

About the Role

The successful candidate will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver compassionate and holistic care to patients and their families. The ideal candidate will have strong clinical skills, excellent communication skills, and a passion for delivering patient-centered care.

Key Responsibilities
  • Assess patient needs and develop individualized care plans that meet reimbursement guidelines
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team
  • Communicate effectively with patients/families at all levels and with all agency personnel
  • Attend and participate in IDT meetings and quality review as scheduled by agency
Requirements
  • Current RN licensure in state of practice
  • Successful completion of a nursing education from an approved school of nursing
  • Two years of clinical nursing experience and at least one year of hospice nursing experience preferred
  • Knowledge of the framework, function, and organization of a home health and hospice care program
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines
  • 12 hours of continuing education annually
  • CPR certification
  • Ability to accept travel based on agency need and weekend/evening and holiday call assignments
