Liberty Hospice, a leading provider of compassionate care for patients with terminal illnesses, seeks an experienced and dedicated Hospice Registered Nurse (RN) to join our team.

As a key member of our interdisciplinary team, you will play a vital role in coordinating physical care for our patients, working closely with family members, physicians, and other healthcare professionals.

Your primary responsibilities will include:

  • Applying your expertise in nursing theories and concepts to develop personalized care plans
  • Carefully documenting patient information and ensuring compliance with reimbursement guidelines
  • Communicating effectively with patients, families, and healthcare colleagues at all levels
  • Collaborating with the hospice team to ensure seamless delivery of care

To be successful in this role, you will possess:

  • A current RN license in the state of practice, as well as a minimum of two years of clinical nursing experience and one year of hospice nursing experience
  • In-depth knowledge of home health and hospice care programs, including the nurse's role within the patient care team
  • Familiarity with principles and practices of nursing according to the nursing practice act, agency policies, and procedures, and reimbursement guidelines
  • A commitment to ongoing education, with 12 hours of continuing education annually
  • CPR certification
  • The ability to work flexible schedules, including travel assignments, weekend, and evening shifts
