As a skilled Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering personalized, comprehensive care to patients in the comfort of their own homes. Our team is dedicated to putting health first, and we're seeking a compassionate and patient-centered professional to join our ranks.

Key Responsibilities:
  • Provide admission, case management, and follow-up skilled nursing visits to patients in their homes.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Confer with physicians to develop and revise care plans, ensuring seamless coordination of care.
  • Coordinate with interdisciplinary teams, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
  • Monitor patient progress, report to physicians and Clinical Managers, and make adjustments to care plans as needed.
  • Supervise and educate home health aides and personal care workers to ensure high-quality care.
  • Prepare and complete accurate medical documentation, including visit reports, progress updates, and care plan changes.
Requirements:
  • Diploma, Associate, or Bachelor's Degree in Nursing
  • Minimum one year of nursing experience, with a strong background in med-surg, ICU, ER, or acute care
  • Home health experience a plus, but not required
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
