Responsibilities- Provide skilled nursing visits to patients in their homes, assessing their needs and developing personalized care plans to promote optimal health and well-being.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
- Administer medications, perform treatments, and provide education to patients and caregivers on disease management, self-care, and health promotion.
- Conduct thorough assessments, identify potential health risks, and develop strategies to mitigate them, ensuring patients receive the best possible care.
- Document patient progress, update care plans, and communicate with healthcare teams to ensure continuity of care.
- Participate in interdisciplinary team meetings to discuss patient care, share best practices, and address any concerns or challenges.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related field
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational, communication, and interpersonal skills
- A valid driver's license, auto insurance, and reliable transportation
