Job Summary

Amedisys is seeking a skilled Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will provide high-quality patient care and coordinate with healthcare professionals to develop and implement individualized care plans.

Key Responsibilities
  • Conduct comprehensive patient assessments and develop care plans in collaboration with the patient's healthcare team.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
  • Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and social workers, to ensure comprehensive patient care.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient information accurately and thoroughly in accordance with company policies and industry standards.
Requirements
  • Current, unencumbered license to practice as a Registered Nurse in the state(s) assigned.
  • One (1+) year of home health clinical experience as a Registered Nurse (RN).
  • Current CPR certification.
  • Able to assess patient status and identify requirements relative to age-specific needs.
  • Familiarity with general use and functions of the computer, including usernames and password concepts, internet, email, navigation of computer desktop, and online HR services.
