Job Description

Westmoreland Health and Rehab Center is seeking a highly skilled Wound Care Specialist to join our interdisciplinary team. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our patients with acute and chronic wounds.

Key Responsibilities:
  • Conduct thorough wound assessments and develop individualized care plans to promote optimal wound healing and prevent skin breakdown.
  • Utilize evidence-based wound management principles and stay up-to-date with the latest advancements in wound care.
  • Collaborate with our interdisciplinary team to ensure comprehensive care and effective communication with patients and families.
  • Develop and implement skin care programs to prevent skin breakdown and promote wound healing.
  • Provide education and training to nursing staff on wound care best practices and protocols.
Requirements:
  • Graduate of an accredited school of nursing with current state licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Minimum of two (2) years' experience in medical/surgical, rehabilitation nursing.
  • Certified Wound, Ostomy and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN) certification is essential.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to prioritize multiple tasks, manage time effectively, and maintain accurate records.
