Job Summary

We are seeking a highly skilled and dedicated Certified Occupational Therapy Assistant to join our team at InnovAge. As a key member of our healthcare team, you will play a vital role in providing occupational therapy services to our patients.

Key Responsibilities
  • Administer Occupational Therapy: Provide occupational therapy services to patients in accordance with physician orders and plan of care under the direction and supervision of the Occupational Therapist.
  • Develop Treatment Goals: Collaborate with the Occupational Therapist to develop treatment goals and objectives for patients.
  • Adapt Intervention Environment: Adapt the intervention environment, tools, materials, and activities to meet the needs of patients.
  • Interact with Patients and Families: Interact with patients and their families to provide education and support.
  • Prepare Clinical Notes: Assist in preparing clinical and progress notes.
  • Participate in Care Planning: Participate in care planning with other healthcare professionals.
  • Consult with OT: Consult with the Occupational Therapist regarding changes in treatment plans.
  • Prepare Medical Records: Prepare medical records, progress notes, and updates care plans for each patient visit.
  • Inform OT and Team Members: Inform the Occupational Therapist and other team members of changes in patient condition and needs.
  • Continually Reevaluate Needs: Continually reevaluate the needs of patients and families.
  • Consult with OT to Determine Treatment Plan: Consult with the Occupational Therapist to determine whether the treatment plan should be continued, changed, or terminated.
  • Instruction to Patient/Family: Provide instruction to patients and families in home programs with documentation in medical record notes.
  • Complete Progress Notes and Paperwork: Complete and submit progress notes and paperwork in a timely manner per Agency policy.
  • Report Incidents/Variations: Immediately report to the supervising Occupational Therapist any patient incidents/variances or complaints.
  • Participate in Continuous Quality Improvement: Participate in continuous quality improvement activities and program evaluations.
  • Maintain Treatment Areas: Maintain treatment areas, equipment, and supply inventory as required.
  • Enter Equipment into IGCP Inventory: Enter equipment into IGCP inventory by issuing InnovAge number, entering item into DME book, and placing item(s) into stock within 24 hours of equipment delivery.
  • Update DME Book: Update the DME book as needed, ensuring that it has current information.
  • Maintain O.T. Forms: Maintain O.T. forms for use within the department by keeping files stocked.
  • Assist with Data Collection/Evaluation: Assist with data collection/evaluation by maintaining the Log Book on a daily basis.
  • Monitor Own Performance: Monitor own performance and identify areas where additional assistance and/or instruction is needed.
  • Document Participant Care: Document participant care within 24 hours of home visit and complete progress notes on participants receiving regular O.T. care.
  • Respond to Requests for Services: Respond to requests for services by contacting participants either by phone or in person within 48 hours of referral being given.
Requirements
  • Current Certification: Current certification from the National Board for Certification of Occupational Therapy (NBOT).
  • CPR Certification: Current CPR certification or the ability to obtain CPR certification is required.
  • Personal Transportation: Requires personal transportation, state-issued driver's license, good driving record, and auto insurance as required by law.
  • Experience Working with Frail or Elderly: 1+ year of experience working with the frail or elderly.
Preferred Qualifications
  • Experience as COTA: 2+ years of experience as a Certified Occupational Therapy Assistant.
  • Bilingual: Bi-lingual.
