The Department of Developmental Services is responsible for providing services and supports to individuals with developmental disabilities. Our goal is to empower individuals to lead self-determined lives by providing person-centered approaches in a supportive environment.

We are seeking a highly motivated and compassionate Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will provide general behavioral training and psychiatric nursing care to STAR consumers. This role will involve direct care, training, and participation in recreational activities with consumers. Additionally, you will provide cross-training to the consumer's community home staff or family to maintain a continuity of care.

About the Role

This is a challenging yet rewarding role that requires strong communication and interpersonal skills. As a Patient Care Coordinator, you will work closely with consumers, families, and other healthcare professionals to develop and implement individualized care plans. You will also be responsible for maintaining accurate records and reports, as well as participating in ongoing training and professional development opportunities.
