About This Role

We are seeking a skilled Healthcare Coordinator to join our team at Diverse Lynx. As a key member of our healthcare team, you will play a critical role in coordinating the care and discharge of patients, ensuring seamless transitions between healthcare settings.

Job Summary

The ideal candidate will have experience in acute hospital discharge planning, with a focus on patient-centered care and efficient resource management. Your strong analytical skills and ability to communicate effectively with interdisciplinary teams will enable you to navigate complex clinical situations and identify opportunities for improvement.

Key Responsibilities
  • Patient Screening and Assessment: Conduct initial and concurrent screenings to identify moderate or high-risk patients for readmission, and develop comprehensive care coordination plans that meet their clinical needs.
  • Care Planning and Coordination: Collaborate with healthcare teams to formulate transition plans, including cost-benefit analyses of care options, and ensure timely communication with physicians and other stakeholders.
  • Utilization Management: Review medical records daily to ensure patients continue to meet level of care requirements, and work with Physician Advisors and Attending Physicians to obtain necessary documentation for current level of care determination and assignment.
  • Care Transitions: Identify barriers to discharge and work with multidisciplinary teams to expedite care, monitor length of stay, and facilitate discharge.
Requirements

To be successful in this role, you will need:

  • A CA RN License and BLS certification
  • ACLS certification preferred
  • Strong analytical and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
