Key Responsibilities:
  • Conduct thorough assessments of veterans' needs and develop individualized care plans

  • Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive support
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Ensure compliance with Veterans Directed Care (VDC) regulations and guidelines
Requirements:
  • Bachelor's degree in Human Services or a related field
  • Minimum 2 years of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in Microsoft Office and other software applications
