Job Title: Admissions Case Manager

Join Boston Senior Home Care, Inc. as an Admissions Case Manager and play a vital role in ensuring the well-being of our elderly clients. As a key member of our team, you will be responsible for conducting initial home visits to assess eligibility, evaluate needs, and enroll elders in the State Home Care Program.

Key Responsibilities:
  • Conduct home visits to assess, evaluate, and enroll elders for state home care services and/or make recommendations for community assistance.
  • Develop a comprehensive care plan based on individual unmet needs of the elder, ensuring interdisciplinary review.
  • Present new cases at the Admissions Review Team for approval and interdisciplinary guidance.
  • Implement the plan by utilizing Boston Senior Home Care and other community resources.
  • Maintain consumer and care plan monitoring through ongoing vendor communication and phone calls to consumers and informal supports until the case has been transferred to an ongoing case manager.
  • Create the consumer record and complete documentation of all significant communications from vendors, consumers, and informal supports.
  • Adhere to Boston Senior Home Care and EOEA compliance standards regarding timeliness of visits, documentation, and follow-up.
  • Maintain a working knowledge of Boston Senior Home Care and other community programs or have the skill to access this information.
  • Develop and hone skills to advocate for consumers in a wide variety of arenas.
  • Contribute to organizational life through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others.
  • Consult with Team Managers and other staff as needed.
  • Conduct ongoing outreach efforts to ensure access to services for all consumer groups.
Requirements:
  • Sensitivity to elder issues and a commitment to maintaining clients at home with dignity.
  • Ability to effectively communicate in person and by telephone.
  • Must have computer experience and be able to perform basic data entry and effectively utilize MS Office Suite and Boston Senior Home Care's database.
Work Environment:

This job operates in a professional office environment. The incumbent must be able to travel to the community to perform visits at consumers' homes and travel under all weather conditions.

Physical Demands:
  • The employee must be able to carry 15 to 20 pounds.
  • The employee is regularly required to talk and hear.
  • The employee is frequently required to stand, walk, use hands to finger, handle or feel, and reach with hands and arms.
Position Type/Expected Hours of Work:

This is a full-time position, Monday-Friday, 9:00 AM - 5:00 PM.

Travel:

The incumbent must be able to travel to the community to perform visits at consumers' homes and travel under all weather conditions.

Required/Preferred Education and Experience:
  • BS or BA degree required, and licensure preferred.
  • Relevant work experience preferred and sensitivity to elder and diversity issues.
Additional Eligibility Qualifications:

The Admissions Case Manager must show sensitivity to elder and diversity issues and a commitment to maintaining clients at home with dignity.
