RN Case Manager

Key Responsibilities:- Collaborate with the healthcare team to evaluate, predict, and facilitate the trajectory of patient care.
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet the individual's healthcare needs.
- Work closely with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Develop and implement effective care plans that address the unique needs of each patient.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal patient outcomes.

- RN Case Manager experience required.
- Be a licensed RN in the state of Massachusetts.
- Minimum of 2 years Med/Surg experience required.
- Case Manager certificate preferred.
- ADN required, but BSN strongly preferred.
- Acute hospital experience preferred.
