LPN Care Coordinator

We are currently seeking a talented LPN to join our team on a Full Time or Part Time basis. The successful candidate will have the opportunity to work with a dynamic team, utilizing their skills and expertise to provide high-quality patient care. This is an excellent opportunity for individuals who are passionate about making a positive impact in the lives of others.
The ideal candidate will be a graduate of an approved school of nursing, with the ability to observe and identify resident needs, report to the RN/Supervisor, and collaborate with the RN in making clinical decisions. Prior experience working in a skilled nursing facility is preferred but not required. The LPN functions under the direction and supervision of the RN/charge nurse and the Nursing Supervisor.
Key Responsibilities:
- Lead and communicate with the team in the planning and implementation of care
- Coordinate activities of the nursing unit and prioritize tasks as needed
- Collaborate with CNAs, physician, and other disciplines to ensure comprehensive resident care
- Participate in thorough documentation of care in the EMR
The successful candidate can expect a competitive salary of $70,000 per year, plus a generous sign-on bonus of $5,000 for Full Time LPNs. Other benefits include health insurance, retirement savings plan with company match, paid time off, elective holidays, and membership in the 1199 union.
