Job Summary:
ICL is seeking a highly skilled Care Specialist LPN to join our team. As a Care Specialist LPN, you will provide case management services to individuals with serious mental illness and substance abuse disorder. This role involves working closely with an interdisciplinary treatment team to develop and implement care plans that address the unique needs of each individual.

Responsibilities:
  • Provide case management services to individuals with serious mental illness and substance abuse disorder.
  • Develop and implement care plans that address the unique needs of each individual.
  • Work closely with an interdisciplinary treatment team to ensure seamless care coordination.
  • Provide nursing care as assigned by registered nurse in accordance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing.
  • Maintain accurate and timely case records, including weekly progress notes, quarterly service plan reviews, assessments, and medical records.

Requirements:
  • Valid NYS license as Licensed Practical Nurse and current NYS registration.
  • At least one year of experience providing services to individuals with serious mental illness.
  • Ability to work effectively in a team environment.
  • Strong communication and interpersonal skills.
  • Basic understanding of the causes and processes of mental illness and substance abuse disorder.

Salary:
$65,000 - $75,000 per year, depending on experience.
