Licensed Practical Nurse (LPN) Care Coordinator Position

We are looking for a dedicated and compassionate Licensed Practical Nurse to fill the role of Care Coordinator in Brooklyn, New York. As a key member of our healthcare team, you will work directly with patients, families, and healthcare providers to ensure seamless care coordination.

This position requires excellent communication, interpersonal, and organizational skills. You will be responsible for developing and implementing care plans, managing patient records, and providing education to patients and their families on disease management and healthy lifestyle choices.

Essential Responsibilities:

  • Develop and implement individualized care plans in collaboration with healthcare providers, patients, and families
  • Serve as a liaison between patients, families, and healthcare providers to advocate for patient needs and preferences
  • Provide education to patients and their families on disease management and healthy lifestyle choices
  • Track patient progress and coordinate follow-up care
  • Maintain accurate and up-to-date patient records, care plans, and reports

Compensation and Benefits:
