About the Role:
PAM Health, LLC is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring the seamless coordination of patient care across various healthcare settings.
Key Responsibilities:
- Conduct thorough reviews of inpatient admission records to optimize hospital resource utilization and determine the most appropriate level of care.
- Assess and identify discharge needs, developing and implementing effective discharge plans that meet the unique requirements of each patient.
- Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes, fostering a culture of excellence and patient-centered care.
- Maintain and enhance relationships with payors, ensuring seamless reimbursement and minimizing administrative burdens.
Requirements:
- Licensure in the state where the hospital resides is preferred.
- Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
- Current BLS certification is required.
- Three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
What We Offer:
- A competitive salary range of $80,000 to $120,000 per year.
