Overview:
PAM Health, LLC is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in the interdisciplinary coordination of care for a designated patient population.

Your responsibilities will include reviewing inpatient admission records to ensure proper utilization of hospital resources and determining admission for the appropriate level of care. You will also assess and identify discharge needs, coordinating an appropriate discharge plan.

In this role, you will work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes. Additionally, you will maintain and enhance payor relationships.

Responsibilities:

* Perform reviews of inpatient admission records
* Determine admission for the appropriate level of care
* Assess and identify discharge needs
* Coordinate an appropriate discharge plan
* Work collaboratively with the interdisciplinary team
* Maintain and enhance payor relationships

Qualifications:

* Licensure in the state where the hospital resides is preferred
* Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker
* Current BLS certification required
* Three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting

Pay Range:
USD $65,000.00 - USD $85,000.00 per hour
