Job Overview:
Therapeutic Alternatives is a leading provider of mental health services, and we are currently seeking a qualified professional to join our Hospital Transition Team. In this role, you will work closely with recipients, families, and healthcare providers to ensure seamless transitions and optimal outcomes. Your expertise in crisis management, treatment planning, and community coordination will be invaluable in this position.

Duties and Responsibilities:
As a Hospital Transition Team Qualified Professional, you will be responsible for providing crisis response, treatment planning, and one-on-one intervention with communities. You will also coordinate movement across levels of care, facilitate discharge planning, and promote community re-entry. Additionally, you will work collaboratively with healthcare providers to ensure timely and effective services. Your strong communication skills and ability to work effectively in various settings will be essential in this role.

Qualifications:
To be successful in this role, you must have a minimum of one year's experience in providing crisis management services in assertive outreach, assertive community treatment, emergency departments, or other service-providing settings. You must also possess a valid NC Driver's License and have had at least 8 years of driving experience. Furthermore, you should hold a license, certificate, registration, or permit issued by a governing board regulating a human service profession. A Master's degree in a human service area with one year of experience working with individuals within disciplines after the degree is preferred.

Benefits: We offer a comprehensive benefits package, including health insurance, retirement plans, and paid time off. Our competitive salary range is $60,000 - $80,000 per year, depending on qualifications and experience.
