We are seeking a highly skilled Licensed Vocational Nurse to join our team at the Substance Abuse Treatment Facility in Corcoran, CA. As a Licensed Vocational Nurse, you will play a critical role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Assist in the implementation of the CalAIM for Justice Involved (JI) Initiative, Whole Person Care Program (WPCP), and Complete Care Model (CCM) by integrating guidelines and protocols into the pre-release planning process.
- Collaborate with the healthcare team and other internal and external stakeholders to ensure the delivery of appropriate and timely pre-release health care services to eligible patients.
- Address the ongoing pre-release needs of patients and provide individualized care to meet their unique needs.
- Work closely with the Nurse Consultant III (Supervisor) and Supervising Registered Nurse II (SRN II) to ensure seamless communication and coordination of care.
- Participate in training and development opportunities to enhance your skills and knowledge in providing high-quality patient care.
- Possession of a valid, active license from the State of California to practice as a Licensed Vocational Nurse.
- Ability to work collaboratively as part of a healthcare team to provide high-quality patient care.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
