About You

The ideal candidate for the Outreach Specialist role should possess the following qualifications:

Education:
A Bachelor’s degree in healthcare, public health, social work, or a related discipline (or equivalent professional experience).

Experience:
Comprehensive knowledge of New York State's Medicaid program and Medicaid Advantage Plan. Previous experience in community outreach, healthcare enrollment, or public assistance programs is preferred.

Entrepreneurial Spirit:

Communication Skills:

About the Role


The Outreach Specialist will report to the Director of Care Management, responsible for providing strategic insights, sound judgment, organization, and evidence-based analysis to guide decisions and meet CINQCARE's objectives.

The Outreach Specialist should embody CINQCARE's core values: Trust, Empathy, Commitment, Humility, Creativity, and Community Focus.


The Outreach Specialist for New York State's Medicaid Advantage Plan (MAP) plays a vital role in promoting and facilitating enrollment in the MAP program.

The primary responsibilities of this position include engaging with the community, raising awareness about MAP, and assisting eligible individuals in understanding and enrolling in the program.

This role necessitates a solid understanding of Medicaid policies, outstanding communication and interpersonal skills, and a commitment to enhancing healthcare access for New York residents.

Key Responsibilities

The Outreach Specialist will be tasked with the following:

  • Develop and implement outreach strategies to connect with eligible populations statewide.
  • Participate in community events, health fairs, and meetings to disseminate information and address inquiries regarding the MAP program.
  • Collaborate with local organizations, clinics, and healthcare providers to broaden outreach initiatives.
  • Educate potential beneficiaries about the benefits, eligibility criteria, and coverage options available under MAP.
  • Provide clear guidance on the application process for MAP and assist individuals throughout the enrollment journey.
  • Create and distribute educational materials, including brochures, flyers, and presentations.
  • Support individuals in completing MAP applications, ensuring all necessary documentation is gathered.
  • Help applicants understand their rights and responsibilities within the program.
  • Offer assistance in resolving any challenges encountered during the enrollment process.
  • Maintain accurate records of outreach activities, including the number of individuals reached and assisted.
  • Compile and submit regular reports on outreach efforts and enrollment statistics to the relevant department.
  • Collaborate with internal and external stakeholders to ensure a coordinated approach to outreach and enrollment initiatives.
  • Stay informed about changes in Medicaid policies and program updates, attending relevant training sessions as needed.

Work Environment and Physical Requirements

This position entails both fieldwork and office-based responsibilities.

Frequent travel is required for home visits and to various healthcare facilities, community partners, and offices, regardless of weather conditions.

In-office work is conducted in a standard office environment with climate control, artificial lighting, and an open workspace.


Successful candidates must be able to travel frequently by car and/or public transportation, communicate effectively with customers, vendors, management, and colleagues, sometimes in challenging situations.

Regular use of telephone and email for communication is essential. Extended periods of sitting may be necessary. Candidates must be able to comprehend and prepare documents, with occasional lifting of up to 30 lbs. required. Good manual dexterity for operating common office equipment such as computers, calculators, copiers, and fax machines is necessary. Strong reasoning abilities are important for understanding and utilizing management reports, memos, and other documents to conduct business.
