Home Health RN Case Manager

We are seeking a highly skilled and compassionate Home Health RN Case Manager to join our team at Professional Case Management. In this rewarding role, you will have the opportunity to make a meaningful difference in the lives of our clients.
Job OverviewThe Home Health RN Case Manager will provide high-quality nursing care and case management services to patients in their homes. This position requires strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care.
Responsibilities- Conduct comprehensive assessments of patients' medical and social needs
- Develop and implement individualized care plans to meet patients' goals and objectives
- Provide direct care and education to patients and their families
- Collaborate with interdisciplinary teams to ensure seamless care transitions
- Document all interactions and interventions in a timely and accurate manner
- Graduate of an accredited nursing program
- Current RN licensure in good standing
- Minimum 2 years of home health or related experience
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
We offer a competitive salary of $40 per hour, plus shift differentials where applicable.
