Atrium Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our healthcare system.

About the Role

This position involves providing pharmaceutical care to patients, including reviewing medication lists, identifying potential issues, and making recommendations to improve patient outcomes. You will work closely with other healthcare professionals to develop and implement individualized medication plans that meet the unique needs of each patient.

Key Responsibilities
  • Review medication lists and identify potential issues, such as duplicate therapies, drug interactions, and medication errors.
  • Develop and implement individualized medication plans that take into account a patient's medical history, allergies, and other factors.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and their families on the safe and effective use of medications.
  • Maintain accurate and up-to-date records of patient interactions, including medication lists and treatment plans.

Education and Experience

Bachelor's Degree in Pharmacy or a PharmD from an accredited school of pharmacy is required. Licensed in the state of North Carolina or eligible for reciprocity is also required. Previous hospital pharmacy experience is preferred.
