About the Role

Atrium Health is seeking an experienced Registered Nurse to serve as a Clinical Care Coordinator in our post-acute home health setting. This role requires strong communication and organizational skills, with the ability to prioritize multiple tasks and manage a caseload of patients.

Responsibilities

  • Serves as the primary point of contact for assigned patients, responsible for coordinating their care and ensuring timely delivery of services.
  • Collects data from patient assessments, telephonic contacts, and EMR information to identify potential issues and develop effective resolution strategies.
  • Analyzes data to determine high-risk patients and develops plans to mitigate risks and ensure quality outcomes.
  • Prioritizes multiple tasks and manages caseload effectively, ensuring timely delivery of services and meeting patient needs.
  • Develops and implements comprehensive plans of care, working collaboratively with multidisciplinary teams to promote health and safe outcomes.

Requirements

To be successful in this role, you will need a Graduate degree from an accredited School of Nursing and current RN licensure or temporary license as a Registered Nurse Petitioner in the state where you work and reside. BLS certification is also required. Additional education, training, certifications, or experience may be necessary within the department by the nurse leader.
