Gentiva Hospice is a leading provider of hospice care services, and we are seeking a compassionate and skilled Hospice RN/Care Manager to join our team.

- $85,000 - $110,000 estimated annual salary, based on experience and location
The ideal candidate will have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology, with a strong commitment to providing exceptional patient care. If you share our passion for delivering compassionate and personalized care, we encourage you to apply.
ResponsibilitiesThe successful candidate will be responsible for:
- Providing direct patient care services to hospice patients and supportive care to their families
- Maintaining primary control and professional management of each patient's care plan
- Serving as the primary liaison between physicians, patients/families, and the hospice team
- Empowering patient caregivers by educating them on care for their loved ones
- Managing and collaborating with an interdisciplinary team to provide comfort, peace, and dignity
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care
- Maintaining accurate and comprehensive clinical records
- Participating in company quality assessment and performance improvement programs
To be considered for this role, you must:
- Hold a current RN license in the state of requested employment (or eligibility to obtain)
- Have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
- Demonstrate flexibility and composure while supporting others in crisis situations
- Possess excellent communication and interpersonal skills
- Be comfortable providing care wherever the patient calls 'home'
