About the Role

Gentiva Hospice is a leading provider of hospice care services, and we are seeking a compassionate and skilled Hospice RN/Care Manager to join our team.

Job Summary

  • $85,000 - $110,000 estimated annual salary, based on experience and location

The ideal candidate will have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology, with a strong commitment to providing exceptional patient care. If you share our passion for delivering compassionate and personalized care, we encourage you to apply.

Responsibilities

The successful candidate will be responsible for:

  1. Providing direct patient care services to hospice patients and supportive care to their families
  2. Maintaining primary control and professional management of each patient's care plan
  3. Serving as the primary liaison between physicians, patients/families, and the hospice team
  4. Empowering patient caregivers by educating them on care for their loved ones
  5. Managing and collaborating with an interdisciplinary team to provide comfort, peace, and dignity
  6. Evaluating patient and family needs and assisting in the preparation and execution of the plan of care
  7. Maintaining accurate and comprehensive clinical records
  8. Participating in company quality assessment and performance improvement programs
Requirements

To be considered for this role, you must:

  • Hold a current RN license in the state of requested employment (or eligibility to obtain)
  • Have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Demonstrate flexibility and composure while supporting others in crisis situations
  • Possess excellent communication and interpersonal skills
  • Be comfortable providing care wherever the patient calls 'home'
