Position Overview:

The Patient Service Manager plays a crucial role in overseeing clinical operations and serves as a vital resource for both agency personnel and management. This position involves assessing the quality of patient care provided by nursing staff, whether employed directly or through contractual agreements.

Key Responsibilities:

  • Guide staff in executing home care nursing responsibilities in compliance with Joint Commission, NYS DOH, and CMS standards.
  • Engage in continuous evaluation of nursing and interdisciplinary staff competencies.
  • Collaborate with various departments to support educational and professional development goals.
  • Conduct comprehensive Home Care orientation for interdisciplinary team members and support personnel.
  • Assess the performance of newly hired nurses at the conclusion of their orientation and probationary periods.
  • Implement audits and surveillance measures to ensure ongoing readiness for evaluations.
  • Participate actively in all preparation activities for surveys.
  • Propose and execute quality enhancement initiatives to uphold superior care standards.
  • Monitor incidents, complaints, and concerns, coordinating responses to enhance customer service.

Qualifications:

  • Current licensure and registration as a Registered Professional Nurse in New York State.
  • Bachelor's Degree in Nursing from an accredited institution is required; a Master's Degree is preferred. OASIS Certification and Home Health Coding Certificate are advantageous.
  • A minimum of two years of experience as a community health nurse within a Certified Home Health Agency.
  • At least one year of experience in a supervisory role is preferred, or an equivalent combination of education and experience.
  • Possession of a valid driver's license and the ability to provide personal transportation for home visits during work hours.
